REINSTATE
(this means the license was not renewed timely). A licensee may reinstate an expired license without re-examination for up to 4 years from the last valid expiration date, subject to (1) compliance with applicable CE requirements for period of non-licensure (2) payment of all past due renewal fees and (3) payment of a reinstatement fee of $150. If you are within 2 years
of the last valid expiration date, If you are unable to reinstate electronically but still within 4 years of the last valid expiration date, contact the Commission for a mail-in reinstatement application. A license which is expired for more than 4 years cannot be reinstated by paying back fees and continuing education. Contact the Commission for current education and exam requirements. |
